Shutter Island falls several magnitudes short of Lost Highway. Also, the themes of these two movies aren't really related at all. Bill Pullman's character isn't going crazy, he's grappling with his impotence in a symbolic bizzarro fantasy. There's a similar thing going on in Mulholland Drive and Blue Velvet.
